These Terms of Use (“Terms”) govern your access to or use of the applications, websites, content, products, and services (collectively, “Services”, as more fully defined in section 4) made available by Honest Care, part of Destiny Labs LLC (“Honest Care”, “us”, “we”). PLEASE READ THESE TERMS AND CONDITIONS OF USE CAREFULLY, AS THEY CONSTITUTE A LEGAL AGREEMENT BETWEEN YOU AND HONEST CARE.
By accessing or using the Services, you confirm your agreement to be bound by these Terms, including the information set forth in our privacy policy, located here. Supplemental terms may apply to certain Services, such as policies for a particular event, program, activity or promotion, and such supplemental terms will be disclosed to in connection with the applicable Service(s). Supplemental terms are in addition to, and shall be deemed a part of, the Terms for the purposes of the applicable Service(s). Supplemental terms shall prevail over these Terms in the event of a conflict with respect to the applicable Services.
If you do not agree to these Terms, do not access or use the Services. These Terms expressly supersede prior agreements or arrangements with you. Honest Care may immediately terminate these Terms or any Services with respect to you, or generally cease offering or deny access to the Services or any portion thereof, at any time for any reason, except where prohibited.
Honest Care may amend the Terms from time to time. Amendments will be effective upon Honest Care's posting of such updated Terms at this location or in the amended policies or supplemental terms on the applicable Service(s). Your continued access or use of the Services after such posting confirms your consent to be bound by the Terms, as amended.

2. No Medical Advice by Honest Care
The contents of the Services are for informational purposes only. The content does not provide medical advice and is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health professionals or providers with any questions you may have regarding a medical condition or medicine. Never disregard professional medical advice or delay in seeking it because of the Services.
The contents included with Services, including, without limitation, any text, copy, audio, video, photographs, illustrations, graphics, and other visuals, is for informational purposes only and does not constitute professional medical advice, diagnosis, treatment, or recommendations of any kind by Honest Care.
All information provided by Honest Care, or in connection with any communications supported by Honest Care, is intended to be for general informational purposes only, and is in no way intended to create a provider-patient relationship as defined by state or federal law. While Honest Care Services facilitates your selection of Providers, Honest Care does not provide medical services, and all provider-patient relationship is between you and the Provider.

Reliance on any information provided by Honest Care, Honest Care employees or Honest Care Services is solely at your own risk.
3. Eligible Users
The Services are offered and available to users who are 18 years of age or older. By using the Services, you represent and warrant that you are of legal age to form a binding contract with Honest Care and meet all of the foregoing eligibility requirements.
The Services are offered and available to users who reside in the United States or any of its territories or possessions. If you do not meet all of these requirements, you must not access or use the Services.
By using the Services, you provide your explicit consent to the collection, storage and processing of your personal data as described in our Privacy Policy. You understand and agree that your personal data may be transferred to and stored in the United States or other countries outside of the European Economic Area (EEA) where data protection laws may differ.
4. The Services (“Services”)
I understand the services include the completion of an online assessment specifically designed to gather appeal evidence for health insurance cases related to GLP-1 medications (“Assessment”). The Assessment facilitates the collection of my medical history and related information through its online questionnaire. After completing the Assessment, one of our trained staff members, referred to here as an appeals assistant, will generate a tailored appeal letter and a comprehensive supporting evidence report in PDF format (“Services”).

The Services also provide information about links to services with which you may provide you with online visits for many popular health conditions, which visits may include medical advice from medical professionals who are able to prescribe medication. To the extent you wish to use the service, you are receiving advice and prescriptions from third parties and not Honest Care. Any advice or services you receive will be subject to additional terms you will agree to from such third parties.
The inclusion of information and links related to any Telehealth Services is for informational purposes only. HonsetCare is not arranging for any referral to, or making any recommendation of, or referral to, any particular telehealth provider. The inclusion of any telehealth provider’s information or links in the Services does not constitute an endorsement or recommendation by Honest Care. It is your responsibility to select a telehealth provider of your choice, if any, based on your own research and due diligence. Honest Care does not credential or certify healthcare practitioners. You are completely free to choose any telehealth provider referenced by the Services, to choose your own telehealth provider not referenced by the Services, or to have an exam performed by an in-person provider.
Honest Care makes no representations whatsoever about any other website that you may access through the Services. When you access a non-Honest Care website, please understand that it is independent from Honest Care, and that Honest Care has no control over the content on that website. In addition, a link to a non-Honest Care website does not mean that Honest Care endorses or accepts any responsibility for the content, or the use, of the linked site. It is up to you to take precautions to ensure that whatever you select for your use or download is free of such items as viruses, worms, Trojan horses, and other items of a destructive nature.
